Elwood Care Center, Assisted Living & Rehabilitation was a project of the village of Elwood. We opened our doors in 1972. At that time, we employed 18 people. 2002 saw the addition of our assisted living facility. Key Rehabiliation later partnered with Elwood to provide our physical, occupational, and speech therapies. ​​​​​​​​​

2022 marked our 50th anniversary. The Center now has over 60 employees and continues to be an entity of the Village of Elwood.  We have 43 skilled nursing home beds and 8 assisted living units.

We strive to service our residents like we would our own families.  We honor their choices from how they decorate their rooms to which physician they doctor with.  Elwood provides an array of healthcare services  to ensure residents' cares are met at every state of life. 

Our vision is to provide a safe, home-like environment enriched in love, peace, and dignity with the mission that every person who enters through our door is important to us and will be treated with the utmost respect.
